We coordinate complex care.
PCM helps individuals, healthcare professionals, institutions, and public programs organize complex care and support successful living in the community — through government-funded programs and private-pay services, from assessment through ongoing support.
Two ways PCM supports complex care
Public Programs
Service Coordination through New York's Medicaid Waiver programs, designed to help individuals with complex needs live safely and successfully in the community.
- Nursing Home Transition and Diversion (NHTD)
- Traumatic Brain Injury (TBI)
Private Pay
Structured, non-clinical support for individuals and families navigating complex healthcare and community-based situations.
- Non-Clinical Assessment & Care Coordination
- ViaDoma | The Journey Home — hospital-to-home navigation

Common questions
What does PCM actually do?
PCM takes responsibility for the logistics, coordination, follow-through, and documentation surrounding complex care — so nothing important gets lost between hospitals, providers, home services, benefits, and family members.
Is PCM a medical provider?
No. PCM provides non-clinical care management, navigation, and oversight — not medical treatment or hands-on home care.
Is ViaDoma a separate company?
No. ViaDoma | The Journey Home is PCM's hospital-to-home navigation program, delivered under the PCM organization.
What areas does PCM serve?
PCM supports individuals and families across New York State, with deep experience Downstate, in the Hudson Valley, the Capital Region, and surrounding areas. Medicaid waiver work is statewide.
